Set up sign in questions for employees and contractors working alone. You need the Administrator user role to access these settings.
How it works
When contractors and employees sign in, you can ask them to answer these basic questions:
- Which area or zone they're working in
- Expected duration onsite
- if they're working alone
- Who is their safety operator
- What is the purpose of the visit
For each section of the questionnaire, choose if the questions should be:
- Activated – turn on the question
- Required – make answering this question mandatory
- Host notification – include the answer to the question in the arrival notification
Set up the questionnaire
- Go to Locations, then select the location from the drop-down list.
- Select Sign In/Out Questions.
- Select the Contractors tab or Employees tab.
- In the Visit Options tab, select the Activate checkbox next to the Area/Zone Selection question. Choose if this question should be mandatory, and the answer should be included in the arrival notification.
- If you want employees or contractors to enter the zone as free text, select the Activate checkbox next to Area/Zone Other.
- Select the Health and Safety tab.
- In the Expected Duration On-site section, select the Activate checkbox. Choose if this question should be mandatory, and the answer should be included in the arrival notification.
- In the Working Alone section, select the Activate checkbox. Select if this question should be mandatory, and the answer should be included in the arrival notification.
- In the Safety Operator section, select the Activate checkbox. Choose if this question should be mandatory and the answer should be included in the arrival notification.
- Click Save.
- (Contractors only) Select the Other Options tab. In the Purpose of Visit section, select the Activate checkbox. Choose if this question should be mandatory, and the answer should be included in the arrival notification.
- Click Manage Options to add the purpose of visit options.
- Click Save.
