Create a Pandemic Questionnaire to identify at risk visitors

Create a Pandemic-centric visitor sign-in questionnaire for Kiosk users. The visitors responses are used to identify visitors that should be prohibited entry to your location.

For example:  A visitor sign-in questionnaire workflow might look like this.

This article assumes you know how to create a Custom Question for visitor sign-in. If not; please read our excellent article on creating Custom Questions here. However; we do take you through the specific steps below for creating the workflow above.

You will:

  1. Create a new Questionaire (A questionnaire can consist of one or many questions. Our questionnaire will consist of 2 questions).
  2. Create Question 1: The first question will ask visitors if they returned from any of the specifically listed countries in the last 14 days. You will also allow an option for the visitor to state they did not visit any of the listed countries. We will do this by creating an option in the country list titled: None'.
  3. Create Question 2: The second question is only presented if the visitor selects 'None' to the first question.
  4. Add the new Questionnaire you created to those kiosk (s) setup within the location.

You will then be able to report on how your visitors responded to these questions.

1: Create a new Questionnaire

Quick step by step to create the workflow illustrated above.

  1. Go to Tools > Locations.
  2. Click View next to your location.
  3. Select Questionnaire Manager from the left-hand menu.
  4. Select the Kiosk Custom Questions tab along the top.
  5. Select Create a new Questionnaire.
  6. Enter a Name for your Questionnaire (for example "Pandemic Visitor Sign-in Policy").
  7. Select 'Visitor' as your Questionnaire Target Audience.
  8. We recommend you select 'Every Visit' for Show Questionnaire
  9. Select Save and Close.

2: Create Question 1

  1. Select the green 'Create a new Question' button
  2. In the 'Question' field enter your question. For example: "In the last 14 days have you returned from any of the following countries?"
  3. For Question Type select Multichoice.
  4. Select Answer Share only if you want the visitor's host to be informed of how the visitor answered this question.
  5. Enable Compulsory Question.
  6. Select the green 'Add an Option' button found alongside 'Options Available'.
  7. Enter the name of the first country then select Add an Option; then repeat adding each country you wish to include as an option.
  8. Add 'None' as an option, and select the small tick box alongside 'None'. None is required to be added to the country list so that visitors that 'have not' visited any of the countries you list can choose an option that applies to them.
  9. Select Save and Close.

You will now be presented with the Questionnaire homepage for this questionnaire and you will see your first question presented on the screen. You can edit your question any time.

3: Create Question 2

  1. Select the green 'Create a new Question' button
  2. In the 'Question' field enter your question. For example: "In the last 21 days have you suffered from any flu like symptoms?"
  3. For Question Type select Single Choice.
  4. Select Answer Share only if you want the visitor's host to be informed of how the visitor answered this question.
  5. Enable Compulsory Question.
  6. Select the green 'Add an Option' button found alongside 'Options Available'.
  7. Enter Yes then select Add an Option
  8. Enter No
  9. In Parent Question select: In the last 14 days have you returned from any of the following countries? =>None. Note: This question is only presented to those visitors that select 'None' to question 1. 
  10. Select Save and Close. You will now be presented with the Questionnaire homepage and you will now see both your first and second questions presented on the screen. You can edit either question any time.
  11. Select Back to Questionnaire Manager located in the top left of the screen
  12. You will now be presented with a list of your Custom Questionnaires.

4: Add the new Questionnaire on your kiosk(s)

  1. SelectSign In/Out Kiosks from the left-hand menu.
  2. Click View next to your Kiosk.
  3. Select the Question Rules tab.
  4. If you have not added a Rule From Offsite to Onsite, please do so now.
  5. Select the blue View button alongside your new rule.
  6. Select the Kiosk Customs Questions tab that now appears
  7. You will be presented with a list Custom Questionnaires in the box on the right-hand side of the screen labelled Available..
  8. Drag your new Questionnaire from the  list of questionnaires in the 'Available' box to the box on the left titled: 'Enabled at this location'.
  9. Select Save and Close.

If Answer Share was enabled when creating question 1 and 2 above then the visitor's responses are shared with their 'host'. 

Reports showing how visitors responded to your questionnaire are available under Custom Questionnaire Reports. Reports are accessble by your Report Users and found in the upper toolbar of the Dashboard.

Note: If you wish to deploy the new questionnaire to other kiosks also set up at the same location, you must repeat steps 2 to 8 above.
