Create a pandemic questionnaire to identify at risk employees and guests

How it works

To create a pandemic questionnaire, you need to:

1. Create a new questionnaire. This questionnaire consists of two questions.
2. Create your first question. This question asks if the visitor is fully vaccinated against COVID-19.
3. Create a second question. This question asks if the visitor has experienced flu-like symptoms. It is only presented if they select Yes to the first question.
4. Activate the questionnaire so it can be used when people sign in.
5. If you'd like to screen everyone entering your location, clone the questionnaire, then set it up so different types of guests answer the same health screening questions. 

Example visitor sign-in questionnaire workflow 

Step 1: Create a new questionnaire

1. Go to Locations, then select the location from the drop-down list.
2. Select Sign In/Out Questions.
3. Select the Custom Questionnaires tab.
4. Click Create a New Questionnaire.
5. Enter a name for the questionnaire (for example Pandemic visitor sign-in policy).
6. Select your target audience: Visitor, Contractor, or Employee. 
7. Choose how often you'd like to present the questionnaire. We'd recommend every visit for visitors and contractors. 
8. Click Save.

Step 2: Add your first question

1. Click Add Question.
2. In the Question/Title field enter your question. For example: "Are you fully vaccinated against COVID-19?"
3. For Question Type, select Single choice.
4. Select the checkbox next to Host notification. The host will be informed of how the guest answered this question.
5. Select the checkbox next to Compulsory Question.
6. Click Add an Option, the enter Yes.
7. Click Add an Option, the enter No.
8. Click Save and Close.

You will now see the home page for this questionnaire. You can edit your question/s from this screen at any time.

Step 3: Add the second question

1. Click Add Question.
2. In the Question/Title field enter your question. For example: "Have you experienced any flu-like symptoms in the last 14 days?"
3. For Question Type select Single Choice.
4. Select the checkbox next to Host notification.
5. Select the checkbox next to Compulsory Question.
6. Click Add an Option.
7. Enter No.
8. In the Parent Question field, select: Are you fully vaccinated against COVID-19? =>Yes. This question is only presented to those guests that select Yes to question one. 
9. Click Save and Close.

Step 4: Activate the questionnaire

You need to activate the questionnaire so it can be used to sign in to your location. There are no restrictions around activating a questionnaire for sign in/out kiosks, Sign In/Out Manager, or OnLocation Mobile. You can activate it anywhere that you want it to appear. OnLocation Mobile is only available for employees and contractors. 

To activate the questionnaire:

1. Click the current status of the questionnaire.
2. Click Activate.
3. Select where you want the questionnaire to appear (kiosks, OnLocation Mobile, Sign/In Out Manager).
4. Click Save.

Step 5: Clone then update the questionnaire

If you'd like the questionnaire to apply to all types of people visiting your location, you'll need to set up additional questionnaires for these audiences. The easiest way to do this is to clone the questionnaire you just created.

To clone a questionnaire:

1. Click Back to Sign In/Out Questions
2. Click Clone next to the questionnaire, then click Confirm.
3. Click Manage next to the cloned questionnaire, then click Update Questionnaire Details. 
4. Enter a new name for the questionnaire.
5. Select your target audience. Choose a different audience than the one you selected for your first questionnaire.
6. Select how often you'd like to present the questionnaire. 
7. Click Save.
8. Click the current status of the questionnaire.
9. Click Activate, then select where you want the questionnaire to appear.
10. Click Save.

Repeat the above steps if you'd like to set up a questionnaire for the remaining audience type.
