Create a pandemic questionnaire to identify at risk employees and guests

Create a pandemic-centric sign-in questionnaire for people signing into your location. The responses are used to identify anyone who should be prohibited from entering. You can use custom questionnaires on sign in/out kiosks, WolMobile and Sign In/Out Manager. 

How it works

To create a pandemic questionnaire, you need to:

  1. Create a new questionnaire. A questionnaire can consist of one or many questions. This questionnaire consists of two questions.
  2. Create your first question. This question will ask if they returned from any of the listed countries in the last 14 days. You will also allow an option for the guest to state they did not visit any of the listed countries. We will do this by creating an option in the country list titled None.
  3. Create a second question. This question is only presented if they select None to the first question.
  4. Activate the questionnaire so it can be used when people sign in.
  5. If you'd like to screen everyone entering your location, clone the questionnaire, then set it up so different types of guests answer the same health screening questions.

Step one: Create a new questionnaire

  1. Go to Tools > Locations.
  2. Click View next to your location.
  3. Select Questionnaire Manager from the left-hand menu.
  4. Select the Custom Questionnaires tab.
  5. Click Create a New Questionnaire.
  6. Enter a name for the questionnaire (for example Pandemic visitor sign-in policy).
  7. Select your target audience: Visitor, Service Provider, or Employee
  8. Choose how often you'd like to present the questionnaire. We'd recommend every visit for visitors and service providers. 
  9. Click Save.

Step two: Create your first question

  1. Click Add Question.
  2. In the Question/Title field enter your question. For example: "In the last 14 days have you returned from any of the following countries?"
  3. For Question Type, select Multichoice.
  4. Select the checkbox next to Answer Share. The host will be informed of how the guest answered this question.
  5. Select the checkbox next to Compulsory Question.
  6. Click Add an Option.
  7. Enter the name of the first country, then click Add an Option. Repeat for each country you'd like to include as an option.
  8. Add None as an option, then select the checkbox next to None. None is required to be added to the country list so that guests that have not visited any of the countries you list can choose an option that applies to them.
  9. Click Save and Close.

You will now see the home page for this questionnaire. You can edit your question/s from this screen at any time.

Step three: Create a second question

  1. Click Add Question.
  2. In the Question/Title field enter your question. For example: "In the last 21 days have you had any flu-like symptoms?"
  3. For Question Type select Single Choice.
  4. Select the checkbox next to Answer Share.
  5. Select the checkbox next to Compulsory Question.
  6. Click Add an Option.
  7. Enter No.
  8. In the Parent Question field, select: In the last 14 days have you returned from any of the following countries? =>None. This question is only presented to those guests that select None to question one. 
  9. Click Save and Close.

Step four: Activate the questionnaire

You need to activate the questionnaire so it can be used to sign in to your location. There are no restrictions around activating a questionnaire for sign in/out kiosks, Sign In/Out Manager, or WolMobile. You can activate it anywhere that you want it to appear.

Note: Wolmobile is only available for employees and service providers. 

To activate the questionnaire:

  1. Click the current status of the questionnaire.
  2. Click Activate.
  3. Select where you want the questionnaire to appear (kiosks, WolMobile, Sign/In Out Manager).
  4. Click Save.

Step five: Clone then update the questionnaire

If you'd like the questionnaire to apply to all types of people visiting your location, you'll need to set up additional questionnaires for these audiences. The easiest way to do this is to clone the questionnaire you just created.

To clone a questionnaire:

  1. Click Back to Questionnaire Manager
  2. Click Clone next to the questionnaire, then click Confirm.
  3. Click Manage next to the cloned questionnaire.
  4. Click Update Questionnaire Details
  5. Enter a new name for the questionnaire.
  6. Select your target audience. Choose a different audience than the one you selected for your first questionnaire.
  7. Select how often you'd like to present the questionnaire. 
  8. Click Save.
  9. Click the current status of the questionnaire.
  10. Click Activate, then select where you want the questionnaire to appear.
  11. Click Save.
  12. (Optional) Repeat the above steps if you'd like to set up a questionnaire for the remaining audience type. 
Reports showing how people responded to your questionnaire are available under Reports > Custom Questionnaire Reports. Reports are accessible to users with the Reports Manager user role.
