Questionnaire Manager

The Questionnaire Manager function is used to select what questions people are asked when they sign into your location. You must be an Administrator to access the Questionnaire Manager.

All questions set up in the Questionnaire Manager are unique to that location. If you have multiple locations then you will need to set the questions up separately for each location.

Please Note: You are unable to change the first two questions asked during sign in: What is your name? Where are you from? As these are required for returning visitor and reporting.

Navigating to Questionnaire Manager

  1. Go to Tools > Locations.
  2. Click View next to the location you want to manage.
  3. Select Questionnaire Manager from the left-hand menu.

There are three options available to assign for most questions. See below for questions where these options do not apply.

Activate - Enables the question for answering.

Required - Must be answered to be able to sign into the location.

Answer Share - The answer is included in the notification email sent to the host announcing their guest's arrival. This only applies to service providers and visitors who select a host upon sign in.

Questions can also be enabled for WolMobile in this section using these same options. This requires the WolMobile Add-on to be activated before the option appears and it is not available for visitors.

Questions with alternate options

ID Verification - The first option in this section is only to Activate, all subsequent questions have full options.

Photo Capture - When enabled, this is automatically required.

Token Options - These have a variety of options to enable or disable which will inform what questions are asked of the token user.

Bypass Summary Screen - Either enabled or disabled. This is not a question which is asked.

Disable Host Remembrance - Either enabled or disabled. This is not a question which is asked.

Visit Options

This section includes options for the visit including host and zone selection.

Host Selection – Ask the visitor if they are visiting someone today. If yes, they are asked to enter the employee’s name that they are visiting. If set to Required, the first step is skipped, and the visitor is taken directly to entering their host’s name.

Please Note: Host Selection is activated by default when creating a new location.

Host Confirmation – When a visitor is returning and able to select their previous details on sign in, this option will force them to either confirm that they are visiting the same host or change hosts.

Area/Zone Selection – Ask the visitor which zone they are signing into. The options displayed will be the zones connected to the selected LACP, if there is only one connecting zone, this question will be skipped.

Area/Zone Selection Other – Gives the visitor the option of selecting an “Other” option when choosing their zone. This will let the visitor enter the zone they are visiting as a free text field and is treated in Kiosk Question Rules as part of “Any Zone”.

Contact Info

This section details basic contact information about your visitor. This is useful to share with their host in case they need to be contacted or there is an emergency situation at your location. This section includes Email Address, Mobile number, Phone number, and Title/Position.

Email Address and Cell Phone number may be used in triggers to send notifications to the visitor, or in WolEvac to verify their safety.

Health & Safety

This section gathers possible health & safety information required.

Evacuation Assistance - Ask if you need information on whether or not the visitor will need help in an emergency.

Expected Duration On-site - Ask if you need to know how long the visitor will be on-site for security reasons. You can use this question as a trigger to send a notification once someone is past their expected duration.

Identity Verification

This section is used when a visitor's ID needs to be verified before allowing on-site. This section must be entered by a Sign In/Out Manager, and if a Kiosk is used to sign in, then the visitor will be presented with instructions to present their ID to the Sign In/Out Manager which is fully customizable. Once ID Verification is selected, more options appear to be selected.

ID Verification Checked - A simple Yes/No is selected by the Sign In/Out Manager.

ID Verification Reference - Use if an ID reference number needs to be entered, e.g. Driver's Licence number.

ID Verification Type - Select from a list of options the type of ID used, this can be changed using the Manage Options button. You can also restrict the types of ID accepted.

The last part of this section is the message the visitor receives when they sign in through a Kiosk.

It appears as a pop-up message once your visitor has answered all questions, and they must select the 'I Understand' button to clear the message. You can customize the heading and body of the message.

Visitor Cards

This section is used for manually entering a visitor ID card number. This is used when you are not issuing printed visitor passes but do wish to record visitor card numbers against the visitor's profile.

The visitor card is recorded using a unique identifier for the card, this is the Visitor Card Number. On sign out, the visitor is reminded to return the card.

The card number can also be used to sign the visitor out. Instead of entering their name on sign out they can enter their visitor card number to be automatically signed out. This means that if the card number is printed as a QR or Barcode on the card, the visitor can simply scan the card to sign out either from the home screen or the sign out screen.

Visitor ID Card Number - When enabled, a Sign In/Out manager will be prompted to enter the Visitor Card number issued to a visitor.

Visitor Self Register - When enabled, a visitor can enter their own Visitor Card number when signing in on the Kiosk.

Photo Capture

This section is to set up the options for capturing visitor's photo upon sign in.

Photo Capture - This must be enabled for photos to be captured upon sign in. Photo capture must also be enabled in the individual Kiosk.

Photo Capture Frequency - How often a photo of the same visitor should be taken. 30, 90, 180, or 365 days.

Photo Retention Policy - How long you would like a visitor's photo to be stored on our servers.

Other Options

This section includes any miscellaneous options which do not have their own category.

Bypass Summary Screen - Enable to skip the Kiosk screen used to confirm the visitor's answers before signing in.

Disable Host Remembrance - Enable to forget a returning visitor's previous host.

Purpose of Visit - Ask your visitor to choose their purpose of visit from a list of options. You can customize the options in the list using the Manage Options button.

Car Parking - Ask your visitor whether they are parked in your car-park. Enabling this provides further options for more details.

Vehicle Registration Number or Make/Model of Vehicle - Enter details about the visitor's vehicle.

Vehicle Parking Space Number - Enable if you want to know the location of the visitor's vehicle using a parking space number.

Basic Questions for Service Providers

Visit Options

This section includes necessary information about who the service provider is visiting and/or where they will be in the location.

Host Selection – Ask the service provider if they are visiting someone today. If yes, they are asked to enter their host’s name. If set to Required, the first step is skipped, and the service provider is taken directly to entering their host’s name.

Host Confirmation – When a service provider is returning, this option will force them to either confirm that they are visiting the same host or change host. If enabled for an Inter-zone Kiosk, it will confirm the host they currently have selected.

Area/Zone Selection – Ask the service provider which zone they are signing into. The options displayed will be the zones connected to the selected LACP, if there is only one connecting zone, this question will be skipped.

Area/Zone Selection Other – Gives the service provider the option of selecting an “Other” option when choosing their zone. This will let the service provider enter the zone they are visiting as a free text field and is treated in Kiosk Question Rules as part of “Any Zone”.

The service provider card is recorded using a unique identifier for the card, this is the Service Provider Card Number. On sign out, the service provider is reminded to return the card.

Token Options

This section is used in conjunction with the Identity Manager Add-on and does not appear unless this add-on is active. If your service providers use tokens to sign in and out of your location, you can control the settings from here.

Enable the options you want service providers to go through when they use a token to sign in to your location. Enabled by default.

Do Not present any questions. Simple Tag on, Tag off - Enable if you do not want to ask your service providers any questions or present acknowledgement notices as the sign in/out.

Present Basic Questions - Will show any basic questions that have been enabled for the service provider.

Present Custom Questionnaires - Will show any custom questionnaires that have been activated on the Kiosk for service provider.

Present Acknowledgement Notices - Will show any Acknowledgement Notices that have been created for service providers to acknowledge.

Present Welcome Greeting - Will show the welcome greeting enabled on the Kiosk for service providers.

Print Pass - Will print a badge pass for the service provider if one is enabled.

Token Self Register - Eable the option for service providers to register their own token to use when signing in and out from the Kiosk. This can be used when a service provider first comes to your location to register their own token e.g. their driver's license.

Quick Scan Tokens and Background Sync - This only appears if the above option is enabled. Enabling this speeds up employee sign in using token scanning by switching your Kiosk into a background sync mode. This will gradually sync all sign in and out information from the Kiosk with WhosOnLocation over time, instead of syncing before allowing the next person to sign in/out.

Please Note: If Quick Scan mode is enabled your employee sign in/out data may be slightly delayed as the information is being synced with WhosOnLocation.

Present Basic Questions - Will show any basic questions that have been enabled for the employee.

Present Custom Questionnaires - Will show any custom questionnaires that have been activated on the Kiosk for employees.

Present Acknowledgement Notices - Will show any Acknowledgement Notices that have been created for employees to acknowledge.
