Questionnaire Manager

The Questionnaire Manager function is used to select what questions people are asked when they sign into your location. You must be an Administrator to access the Questionnaire Manager.

All questions set up in the Questionnaire Manager are unique to that location. If you have multiple locations then you will need to set the questions up separately.

Note: You are unable to change the first two questions: What is your name? Where are you from? As these are required for returning and reporting functionality. If you have questions around this, please submit a request to our team.

To access Questionnaire Manage -

1. Go to Tools > Locations

2. Click View next to the location you want to manage

3. Select Questionnaire Manager from the left-hand menu

There are three main options available for most, but not all, questions.

Activate - When selected the question becomes an option to answer when possible.

Required - When selected the question must be answered to be able to sign into the location.

Answer Share - When selected the answer to the question is sent to the host of the visitor or service provider in the email notifying them of their guest's arrival. This only applies to service providers and visitors who select a host upon sign in.

Some questions also have a WolMobile section. This is to control what questions are asked of people signing in using WolMobile and requires the WolMobile Add-on to be activated before the option appears. This is not available for Visitors.

Basic Questions Visitors

Contact Info

This section includes basic contact information about your visitor which is very useful to share with their host in case they need to be contacted and useful information to have in emergency situations. This section includes Email Address, Mobile number, Phone number, and Title/Position.

Health & Safety

This section is used to gather possible health & safety information required depending on your location.

Evacuation Assistance - Ask if you need information on whether or not the visitor will need help in an emergency

Expected Duration On-site - Ask if you need to know how long the visitor will be on-site for security reasons. You can use this question as a trigger to send a notification once someone is past their expected duration

Identity Verification

This section is used when a visitor's ID needs to be verified before they are allowed on-site. This section must be entered by a Sign In/Out manager, and if a kiosk is used to sign in, then the visitor will be presented with instructions to present their ID to the Sign In/Out manager which is fully customizable. Once ID Verification is selected, more options appear to be selected.

ID Verification Checked - A simple Yes/No which is selected by the Sign In/Out manager.

ID Verification Reference - Use if there is a reference number which needs to be entered from the ID, e.g. Driver's Licence number.

ID Verification Type - Select from a list of options the type of ID used, this can be changed using the Manage Options button. You can also restrict the types of ID which are accepted using this question.

The last part of this section is the message the visitor receives when they sign in through a kiosk.

It appears as a pop-up message for your visitor once they have answered all other questions, and they must select an 'I Understand' button to clear the message. You can customize the heading and body of the message to give exact instructions for your particular location.

Visitor Cards

This section is used for manually entering an ID card number for visitors. This is used when you are not issuing printed visitor passes but do wish to record visitor card numbers against the visitor's profile.

Visitor ID Card Number - When enabled, a Sign In/Out manager will be prompted to enter the ID card number issued to a visitor.

Photo Capture

This section is for setting up the options for capturing visitor's photos upon sign in.

Photo Capture - This must be enabled for photos to be captured upon sign in. Photo capture must also be enabled in the individual kiosk.

Photo Capture Frequency - How often a photo of the same visitor should be taken. 30, 90, 180, or 365 days.

Photo Retention Policy - How long you would like a visitor's photo to be stored on our servers.

Other Options

This section includes any miscellaneous options which do not have their own category.

Disable Visiting Someone - Asking a visitor if they are visiting someone is turned on by default, selecting this option completely disables that question.

Visitor Hosting Mandatory - The visitor must name a host for their visit.

Bypass Summary Screen - Enable this to skip the kiosk screen used to confirm the visitor's answers before signing in.

Disable Host Remembrance - Enable this to forget a returning visitor's previous host.

Purpose of Visit - Ask your visitor to choose their purpose of visit from a list of options. You can customize the options in the list using the Manage Options button.

Car Parking - Ask your visitor whether they are parked in your car park. Enabling this provides further options for more details.

Vehicle Registration Number or Make/Model of Vehicle - Enter these details about the visitor's vehicle.

Vehicle Parking Space Number - Enable this is if you want to know the location of the visitor's vehicle using a parking space number.

Basic Questions for Service Providers

Visit Options

This section includes necessary information about who the service provider is visiting and/or where they will be in the location.

Visiting Someone - Enable this if you want your service provider to select a host for their visit.

Area/Zone Selection - Enable this if you want your service provider to select the zone they are working in.

Area/Zone Other - Enable this if you want to give your service provider the ability to enter the zone they are working in as free text instead of what has been set up for the location. This is useful if you haven't set up zones in this location.

Health & Safety

This section is used to gather possible health & safety information required depending on your location.

Evacuation Assistance - Enable this if you need information on whether or not the service provider will need help in an emergency

Expected Duration On-site - Enable this if you need to know how long the service provider will be on-site for security reasons. You can use this question as a trigger to send a notification once someone is past their expected duration

Working Alone - Enable this is you need to know if your service provider will be working onsite alone for health & safety reasons

Safety Operator - Enable this if your service provider should select a person to be their safety operator from a list of employees and service providers tagged with the 'Safety Operator' role.

Service Provider Cards

This section is used for manually entering an ID card number for service providers. This is used when you are not issuing printed badge passes but do wish to record ID card numbers against the service provider's profile for their visit.

This section is used in conjunction with the Identity Manager Add-on and does not appear unless this add-on is active. If your service providers use tokens to sign in and out of your location, you can control the settings from here.

Token Scan Rule - Enable the questions you want service providers to be asked when they use a token to sign in to your location. You have 4 options to choose from:

  1. Do Not present any questions. Simple Tag on, Tag off
  2. Present Basic Questions
  3. Present Custom Questionnaires
  4. Present Acknowledgement Notices

By default this is set to option 1. But note you can select any combination of options 2, 3, and 4.

Token Self Register - Enable the option for service providers to register their own token to use when signing in and out from the kiosk. This can be used when a service provider first comes to your location to register their own token e.g. their driver's license.

Other Options

This section includes any miscellaneous options which do not have their own category.

Car Parking - Ask your Service Provider whether they are parked in your car park. Enabling this provides further options for more details.

Vehicle Registration Number or Make/Model of Vehicle - Enter these details about the service provider's vehicle.

Sign-in Options

Area/Zone Other - Enable this if you want to give your employee the ability to enter the zone they are working in as free text instead of what has been set up for the location. This is useful if you haven't set up zones in this location.
